Registration/Schedule Adjustment
PLEASE READ CAREFULLY
Dates of registration, schedule adjustment deadlines and withdrawal deadlines are listed in the UAH calendar -- check carefully.
Registration creates a financial commitment to the University and fees must be paid in compliance with the published fee statement policy.
Withdrawal and refund policies are outlined in the UAH catalog and schedule of classes -- read carefully.
This form may NOT be used to add a class after the published "Last Day to Add a Class" date. A student requesting to add a class after the "Last Day to Add a Class" MUST submit a petition to the Office of the Provost clearly stating the reason for the request. The petition must include signed recommendations (approval/non-approval) from the Instructor, Chair of the Department offering the course, the Dean of the College in which the student is enrolled, and in the case of graduate students, the Graduate Dean.
Before making any Schedule Adjustments you should consult with your academic advisor. Read the following statements and respond appropriately on the front of this form.
If you are receiving financial aid, scholarships or loans, contact the Office of Financial Aid.
If you have an assistantship, contact the Office of Graduate Studies.
If you are an international student, contact the International Student Advisor.
If you are a co-op student, contact the Cooperative Education Office.
If you are a student-athlete, contact the Athletic Compliance Officer.
